Information for clients

If you are a participant in the NDIS and your plan identifies the need for certain hearing services, your National Disability Insurance Agency (NDIA) Planner may refer you to the Hearing Services Program (the program). Find out more about what the NDIS means for clients of the program

Information for contracted service providers

NDIS application form

To assist with the NDIA Planners and NDIS participants please complete the application form, and return it to the Office. Please ensure the form is signed by the client, and that a medical certificate is attached. Services cannot be provided until a voucher has been issued to the participants.

The NDIA has launched a suite of communication tools to assist providers and the community to become NDIS ready.

Hearing Services NDIS Transition Plan

A Transition Plan has been developed which incorporates feedback from cross-Agency consultation and the series of Transition Planning Workshops held in 2015. The Transition Plan consists of 11 work-packages that will need to be worked through to support a smooth transition of eligible clients to the NDIS by mid-2019.

Further opportunities for stakeholder consultation on the respective work packages will be confirmed as work packages are progressed.

Audiology Australia National Conference NDIS round table discussion

The Department of Health participated in a round table at the Audiology Australia National Conference on May 24 2016. The conference presentation containing key points about transition of voucher and CSO clients who can access the NDIS to that scheme is available as a PDF version of the conference presentation, and a Word version of the conference presentation.

NDIS transition information sessions

A series of information sessions were held in Canberra, Sydney and Melbourne in June 2015.  The powerpoint as a pdf and as a powerpoint presentation and a summary of the sessions is now available.

NDIS transition planning workshops

A series of Transition Planning Workshops were held in Sydney and Brisbane in August 2015, Melbourne and Perth in September 2015, and Adelaide in October 2015. A summary of the workshops and the powerpoint as a pdf or as a powerpoint presentation is available.
